Athletic Clearance


Every athlete (grades 9-12, new and returning athletes) must complete the online clearance questionnaire on the Permission Click website

and download the following forms (1) physical, (2) authorization for emergency medical care waiver to become eligible to participate in practices, tryouts, or clinics.

1. Please take the Physical Form -Take the Physical Form to your health care provider & have it completed by an MD, DO, PA or NP. MAKE SURE THE DOCTOR SIGNS AND DATES THE FORM.

2. Please fill out the authorization for emergency medical care waiver form. Make sure to sign in ink.  We do not accept electronic signatures.

3. Email both forms to [email protected]

4.You will receive a clearance email from the Athletics Office with a copy of your student's emergency medical care waiver form.. This copy is your receipt. Present this Form to your coach when you attend the first practice, tryout, or clinic.

Due to COVID many student-athletes physicals will remain current and valid during the 2021-2022 school year. All student-athletes will still need to go online and answer the questionnaire on the Permission Click website and email or download a new completed emergency medical care waiver.  Please be sure to note on the emergency medical care waiver that your student has a current physical so that our office can clear your student.



Athletic physicals are required before an athlete can participate on a team (this includes attending tryouts).

Physicals are good for one calendar year. Schedule your physicals for the middle to end of May so it will be valid for the entire next school year and will cover all sports seasons  – Fall, Winter and Spring.

We don’t clear an athlete for a sport unless their physical will be valid for their entire season. Having a physical that will expire during season will result in the athlete not being cleared for that sport until we receive an updated physical.

Physicals can be done by your family doctor or at any of the local walk in clinics.
